ALL TEAMS MUST
submit the following at registration. Any team not complying with these
requirements will NOT
be permitted to participate in the tournament:
- Out-of-state teams must submit a PERMISSION TO TRAVEL
form.
- NOTARIZED MEDICAL RELEASE
forms for all players.
- Only 2009-2010 PASSES for those players listed on roster.
Three (3) guest players (2 for U9 - U10) are permitted and must have a
2009-2010 player pass.
- A COPY of your state certified roster.

print the following 2 forms, complete them and bring to
registration.
