1.  Before completing this application, please be sure that you have reviewed our Tournament Information,  Tournament Rules and Codes of Conduct.

2.  The entry fee is $495 for U11-U18, and $425 for U9-U10. Payment of the entry fee must be received by the Voorhees Soccer Association within 10 days of submitting the online application. Your application will not be considered until your entry fee has been received. Please include your team name, age group, and gender on your check. Checks should be made payable to:

3.  Deadline: All entry fees must be postmarked no later than March 31, 2008.

4. All entry fees will be deposited upon receipt, however this does not constitute acceptance to the tournament. Acceptances will be emailed on or about April 15, 2008.

           I understand and agree if my team is not accepted, the entry fee will be returned in full. I further understand and agree once my team is accepted and later declines, the entry fee is forfeited. If the tournament is cancelled for any reason prior to the start of the tournament, the Voorhees Soccer Association will refund a minimum of 80% of a team’s entry fee.  I have read the tournament guidelines and the tournament rules as stated on the tournament website and agree to abide by such guidelines and rules.
